Trinity Bejarano, RN brings 20+ years of nursing experience and hands-on support. Price Polley, NASM Certified Nutrition Coach, helps you build sustainable nutrition and habit strategies.

Getting started is easy:

- Complete your patient intake form.
- Our Family Nurse Practitioner will review your information and determine eligibility.
- If approved, we will text you with your next steps.
- Complete your initial consultation with our team and choose how you would like to receive your medication:
- Weekly injections at Revive, or
- A patient-specific vial shipped directly to your home.
Your initial consultation includes medication education, potential side effects, injection education when applicable, nutrition guidance, and an opportunity to discuss your goals and treatment plan.
There is no additional office fee for weekly in-office injections.
How Do I Get My Medication?
Once you are approved for our medical weight-management program, you have two convenient options:
Option 1: Weekly In-Office Injections
Come to Revive Innovations once a week and have your injection administered by our clinical team.
