Weight Loss 4-Packs Are Still Available
We apologize for any confusion caused by our recent medication policy updates. We are happy to clarify that established patients may continue receiving 4-packs of prefilled Semaglutide or Tirzepatide syringes after completing our Reasonable Accommodation Request Form.
Simply click the button below to complete the form. Once the form has been completed, you may continue your weight loss treatment as normal.
GLP-1 Weight Loss Medications
Patients may continue to have the following options, as appropriate for their treatment plan:
4-Pack of Prefilled Syringes
Semaglutide and Tirzepatide may be available as a 4-pack after completion and approval of the Reasonable Accommodation Request Form.
Weekly In-Office Injection
You may visit Revive each week to have your injection administered by our medical staff.
Home-Shipped Medication Vial
Patient-specific medication vials may also be ordered through one of our compounding pharmacy partners and shipped directly to your home.
GLP-1 Pricing
Our pricing has been updated to include applicable tax, so the price displayed is the total medication price.

For a 4-pack of prefilled syringes, simply multiply the listed single-injection price by four.
Testosterone
Testosterone will not be available as take-home prefilled syringes.

Peptide therapy is still available!
We offer a variety of peptide options based on individual treatment goals and provider approval. Availability and ordering requirements vary by medication.

Vitamin Injections
Vitamin injections, including:

will now be administered in our clinic by our medical staff.
